Spot-slimming and a heart patients diet?

I am 24 year old male 5'7",145 Lbs with 24% body fat (measured by one of those home scales).  What is the best way to tone down and lose some of the fat.  I am also a heart patient and my cardiologist says that I am in good shape and not to worry too much.  Also I have a double chin that is very irritating (my dad has one too and he is not overweight) is it hereditary?  How do I get rid of it?
Plese keep in mind that I am a full time student and I also work full time, thus I have very little available time for exercise, although I do a nice amount of physical work at my job.

Since you are a heart patient, the only way for you is talking to your cardiologist regarding your diet. I would recommend a South Beach or Mediterranean type proved to be good for your heart.

As to your question about spot-slimming, unless it's about mid-section fat distribution, there's no known ways to lose fat locally. You might want to consider facial gymnastics though to firm the under-chin muscles and work on your posture.
