Gastric sleeve trouble

Question
I had the gastric sleeve performed on Sept. 10th of this year.  I have been having complications every since then with it.  I had problems with the band previously, I have had 2 stents and an egd with dilation. I can kinda get liquids down now but still have some problems with swallowing things as simple as mash potatoes.  Will I every be back to normal? Also my birthday is coming up next month will there be a problem with having a drink?  I also spit alot is that normal?

Answer
Hi Dana:

I'm sorry to hear that you've been through so much--first with the Band, and now the Sleeve.  It's tough for me to say what's going on through an email only, but I'm wondering if there is something about your esophogus which may just be very sensitive to bariatric surgery--due to the difficulties you already had with your Band.  Sometimes, VSG patients also need dilation--which may be the case for you.  I really think you need to go back to your surgeon, and most likely see a gastroenterologist.  For the typical patient, I would probably say to wait and see a bit longer--but with your previous history, you're probably better off being a bit more proactive.  The swallowing issues and spit make me think of some sort of motility issue that you may be facing as well.  Basically, I think they're all related and I do believe that a gastroenterologist could help you.  You might get seen sooner if your surgeon calls for you for the consult.  With your past history, I would think this would be something they were watching regardless.

Be sure to mention if you're also experiencing any sort of reflux, as it's pretty common in the early phases post-op, and many surgeons like to prescribe Nexium or a similar PPI for the first few months.

I can tell you that of people I know who have faced similar problems early on with their Sleeve, nearly all (I can only think of one exception) resolved things by 6 months out--with the one hold out taking nearly a year.  Her sleeve did eventually stretch enough that getting down things became much easier and she was quite happy with her quality of life and weight loss.  So don't fret.

Because of the issues you're having getting down even liquids, please be extra careful to watch for dehydration--if your urine is dark, cloudy, or scant--call your surgeon, as you may need to go in to the ER for IV fluids.

As for the drink, usually moderate alcohol is not an issue for sleeve patients.  As long as you are not experiencing hydration issues at that time, one drink is probably fine.  I would check with your surgeon and gastroenterologist first, though--as I'm not sure if there is anything regarding your condition which would prohibit it.  My guess is they'll say one drink is fine, but check to be sure.

I'm sorry I can't be of more help.  I hope things resolve quickly for you.  Please, do call your surgeon about your issues, though.  If s/he is not being responsive, then I would go straight to a gastroenterologist (via your PCP if you need to for insurance).  With your history, it doesn't make sense to wait and see if things will get better.

Kind Regards,
Karla
