What Causes Exocrine Pancreatic Insufficiency?

Several conditions can lead to exocrine pancreatic insufficiency (EPI). These include:

1. Chronic Pancreatitis: Long-term inflammation of the pancreas, often due to factors like alcohol abuse, gallstones, or genetic conditions, can damage the exocrine cells responsible for producing digestive enzymes.

2. Pancreatic Cancer: As a malignant tumor grows in the pancreas, it may obstruct or destroy the exocrine tissues responsible for enzyme production.

3. Surgery or Trauma: Surgical interventions, such as pancreatectomy (partial or complete removal of the pancreas), or trauma to the abdomen can result in EPI.

4. Cystic Fibrosis: This genetic disorder affects the mucus-producing glands and exocrine organs throughout the body, including the pancreas, leading to impaired enzyme production.

5. Diabetes: While not a primary cause, some individuals with long-standing type 1 or type 2 diabetes can develop EPI due to damage to the nerves controlling pancreatic function (diabetic pancreatopathy).

6. Rare Genetic Conditions: Certain rare genetic syndromes, like Shwachman-Diamond syndrome and Johanson-Blizzard syndrome, can cause congenital EPI.

7. Medications: Some medications, such as proton pump inhibitors (PPIs) used for gastroesophageal reflux disease (GERD), may interfere with the activation of pancreatic enzymes and contribute to EPI.
